Organization’s Characteristics Current strengths The mission statement of the hospital is, “Above all else, we are committed to the care and improvement of human life” (Memorial Health, 2019). Allyson believes the organization stands behind its mission statement in all areas of patient care. In support of this mission, the behavioral health department provides patients with the opportunity to participate in seven group
CONSULTATIVE CHANGE RECOMMENDATIONS 8 sessions per day including goal setting, psychotherapy, recreational therapy, activity therapy, and nursing education. Each patient meets with their therapist individually and is seen by the interdisciplinary treatment team daily. The team works well together and focuses on including the patient in making decisions regarding the care they are receiving. Allyson stated she has worked in multiple behavioral health facilities and feels that MHUMC consistently delivers what other organizations struggle to offer their patients (A. Croxton, personal communication, October 28, 2019). Current weaknesses Despite the intensity of services while patients are in the hospital, Allyson feels there is something lacking in the discharge process leading patients to be readmitted frequently. Usually the readmission is within 30 days of discharge. Upon return to the facility, many patients have expressed they felt they were discharged too soon or they didn’t know what to do when they left and ended up having to come back. Allyson has not had an opportunity to research the actual number of readmission or to begin tracking the reasons people have returned for treatment. “I just feel like we are constantly seeing the same patients over and over. It is like a revolving door,” states Croxton (A. Croxton, personal communication, October 28, 2019). This is the primary reason the department has requested a consultation. Evidence-Based Practice Activities Evidence based practice relies on measures that have been researched and have proven by evidence to provide the best quality outcomes for the patient population. MHUMC utilizes EPIC as their Electronic Medical Record (EMR) platform. EPIC supports Evidence Based Computer Documentation (EBCD). The facility has many protocols and order sets that have been created using evidence-based guidelines such as their Heparin protocols, Insulin Sliding Scale protocols,
CONSULTATIVE CHANGE RECOMMENDATIONS 9 and Clinical Institute for Withdrawal Assessment (CIWA) protocols. Each of these protocols are supported by order sets that are built into EPIC and are linked to EBCD documentation. There is a shared governance committee that reviews all evidence-based protocols and order sets annually to ensure they remain current based on the most recent research.
