Pacifica caters to a wide variety of patients that consists of multi-racial backgrounds and mostly the indigent group. The facility provides a tele-interpreter system and a language interpreter video app for patients, as well as the availability of Hispanic speaking personnel at all times. Educational materials in different languages are provided in the form of pamphlets, or taught by the staff to the patients and their family at the appropriate level of understanding. The facility scored low in the domain of charting and documentation. The patients do not have access to any charting through the patient portal, nor do they have full access to their record unless they request the medical records personnel to print their chart. The Nursing staff or the health provider are usually the only ones that documents patient and family concerns in the (EMR) electronic medical records. Another low score by Pacifica hospital is in the domain of Care Support. Families are not encouraged to participate in rounds, as well as in the change of shift reports. The staff do not voluntarily or routinely provide an update on the patients’ plan of care, unless asked by the patient or family member. Family members are not allowed to stay with the patient during rescue events, instead they are escorted to a waiting room and given an update on the situation by the staff. Lastly, patient or family education on how to activate the rapid response system is not required by the facility and family care team members does not have 24hr visiting privileges. Finally, the domain of Care also scored low in all elements. Everyone is treated with respect throughout their hospital stay, but not treated as partners in their care since there is no routine collaboration between the patient or family members and the healthcare providers or staff. Pacifica hospital with regards to pain management does not manage pain effectively in partnership with the patient and family. Pain assessments are not routinely performed on all patients. Most times patients have to be proactive in requesting pain medications, and with the majority of the patient population being drug users, providers are cautious in prescribing pain medications. As to regards to transition plans, the staff in Pacifica informs the patients and family members prior to the change, otherwise it is rarely discussed in the plan of care. Area of Improvement There are several areas in the Pacifica Hospital assessment tool identified that requires improvement. The hospital is in dire financial need and proper management as seen by the assessment tool results.
