Plan Evaluation [C.2.]
To measure the effectiveness of the development of a heart failure support group for both
patients and caregivers, identification of an evaluation plan is necessary. Evaluating stakeholder
by-in is two-fold. First, the engagement of stakeholders will be seen by their participation in the
development of the support group, including asking questions and giving suggestions. Another
measure of stakeholder by-in is the willingness of their participation to recommend the support
group to patients, agreeing to be a guest speaking, or even become a meeting facilitator. The
most measurable evaluation of whether the logistic strategy is successful is being able to start

meeting as a support group. There are many logistical aspects that must come together for a
support group to take place. To measure how suitable the location, space, time, and group size, a
survey will be given to group members. To measure whether the recruiting of members is
successful with be to take attendance. Also an evaluation of the synergy of the group needs to be
evaluated by the facilitator to determine if group members are listening and supportive. This can
also be evaluated by maintaining group members from one meeting to the next. The best overall
measure on how well a heart failure support group is working is to allow members to openly or
privately share their feelings of how the group is helping them cope as a patient or caregiver.
