CONTINUED TREATMENT AND MANAGEMENT OF AF • Anticoagulant medications reduces the risk of clot formation such as • Aspirin 75 – 325 mg po everyday • Coumadin 2-5 mg po everyday INR 2-3 initial dosage 10 mg po for 2 days • Eliquis 5mg twice a day then decrease to 2.5 mg twice a day • Pradaxa 150 mg twice a day • Xarelto 20 mg daily
CONTINUED TREATMENT AND MANAGEMENT OF AF • The medication that will be focused on to treat AF will be • Coumadin 10 mg daily for 2 days then it will be decrease after lab results (INR) • Coumadin is subclass anticoagulants (Epocrates, 2018) • Side effects • Bleeding, ecchymosis, abdominal pain, nausea/vomiting, diarrhea • Mechanism inhibits vitamin K dependent, coagulation factor synthesis (ll, Vll, lX, X, proteins C and S). • Metabolized in the liver and excreted through urine • Half-life 20-60 hours
CONTINUED TREATMENT AND MANAGEMENT OF AF • Diltiazem CD 120 mg daily • Diltiazem CD subclass is antianginals, calcium channel blockers and non- dihydropyridine (Epocrates, 2018). • Side effects peripheral edema, headaches, dizziness, asthenia, hypotension, orthostatic, bradycardia • Mechanism it inhibits calcium ion influx into vascular smooth muscle & myocardium.
