Involving families early and on a consistent basis with the care plan for their loved one would make the transition to care at home more seamless for both the patient and their families. Area of Improvement (C) The domain chosen from the PFCC Assessment Tool as the area that needs improvement is the Care Support Domain. The element in this domain that will be focused on in regards to improving patient-centered care is changing policy and procedures on the units to encourage family to stay and/or be included in rounds and change of shift report. Making an effort to improve upon this element of the PFCC Assessment Tool would aide in fostering a more complete patient-centered environment for both the patient and family. Changing policy and adopting a new procedure in this area could also give the physicians and nurses broader view of the patient/family dynamic and insight into how to better care for the patient mind, body and soul. Improvement Strategy (C1) The strategy to improve upon and/or make a plan to institute family inclusion during rounds and change of shift report would be to create a multidisciplinary team. This multidisciplinary team would address the omission of families during rounds and change of shift report and aid in creating a plan to rollout a new policy and/or procedure to include families in these activities. This team should include different members of the medical staff, each having a stake in the process, as well as, a unique outlook on how to go about addressing the changes/improvements that need to be made. The members of this team would consist of

A multidisciplinary team approach will aide in addressing the need for change/improvement in this area by bringing different ideas, suggestions and solutions from their areas of expertise. Having staff members from different disciplines will enable the team to come up with a well-rounded plan for improvement that works for the entire staff and most importantly the patient and their family.
