The evaluator must be able to see and hear your
assessments in order to evaluate them so make sure your camera is positioned correctly (and moved as you or your patient moves), the audio and video are clear, and that you verbalize what you are assessing when observing/inspecting (for example mouth – teeth, gums hard & soft palate, uvula, etc.). Document your findings on the CHADF – what you see, hear, and feel. Putting ‘normal’, ‘intact’ or WNL is unacceptable and it will be returned for revisions. The only exception is the Review of Systems (history) section where you are directed to “List findings or check as negative”. If you missed assessing an item on the video, just write ‘not assessed’. Documentation of items not assessed on the video will result in needed revisions. This does not apply to the four sections asking for explanation or description, i.e. Describe the purpose of an Opthalmascope for assessing the eyes, Explain the Weber and Rinne tests, Describe the purpose of the speculum, & Discuss the process for assessing breasts and axilla. You do need to complete each of those on the CHADF, even though they are not a required part of your video. Your assessment findings can be neatly handwritten on the CHADF or typed (highly recommended). c.Review the Bates videos (link in Course tips and in the Course) for any systems you need a refresher on, ex. Cardiac – heart sounds. The Bates Guide (eText in your course) is the source evaluator’s use to determine correct assessment technique. d.Run your paper through spell and grammar check before submitting NOTE: We do not recommend YouTube videos that other students have posted because they contain incomplete and inaccurate assessments. They are not WGU videos.
