as the life of patients are sensitive. In an organization, for example a clinic, it is veryimportant to find problems that can affect the security and health of patients. Manyadverse effects had been a problem in many hospital settings, and many due to
BPPG5miscommunication or the wrong paper work. I believe that it is important find the factorsas it can have consequences as a chain reaction and have a negative outcome in the long-term. By finding a constrain, we are able to find solution and help our system thinking forreference. Using this system thinking will further help to apply the solution to treat anyconstraint as a whole, in this case the whole hospital setting.-Problem Finder: As the same as a constrain, it is also important to go deeper into aproblem and be able to search for it. The 5-Whys is an excellent tool to be able to godeeper into finding the root cause of the problem. This tool is helpful as it not only findsthe root of the cause but also helps us find problems along the way to be able to resolvethem. During the finding of those causes, one might be able to find constrains that affectthe organization. By finding the root cause and finding solutions by system thinking, itwill have an impact on the throughput that can benefit the organization both in short andlong term.References:
