(Gonzalez J.S., et al., 2007) Utilizing this knowledge and Susan’s desire not to be over medicated, Susan’s treatment plan was designed to adhere to a diet, exercise and glucose self-monitoring regime with a ninety- day follow-up. Susan’s treatment plan included a low dose Wellbutrin XR, counseling, diet and exercise program and a glucose self-monitoring regimen. If Susan takes the Wellbutrin as prescribed, this medication can balance the chemicals in her brain to replace her negative behaviors, balance her ability to function during the day and improve her overall self-esteem. Over the next ninety days, Susan followed the treatment plan that was created by her PCP, every day she took her Wellbutrin XL, attended counseling, as well as maintained a diet and exercise program. As Wellbutrin XL is an extended release medication and only taken once a day, it allowed Susan to be able to achieve the benefit of the dual effect of the needed norepinephrine (NE) and dopamine (DA) neurotransmitter balance to properly manage her MDD. (Fava, et al., 2015) Susan reported after the two weeks she noticed a difference in her ability to function and tend to the basic needs of her daughter. As time continued, Susan said through counseling and maintaining a proper diet and exercise program; she had more energy allowing along with a better thought process as she made decisions of the needs she and her daughter required to survive. Through counseling, Susan was able to balance the sadness and thoughts of suicide while learning how to overcome what she realized were the small things in life. Susan’s counseling sessions allowed her to discuss her issues and conditions of MDD while receiving feedback ultimately leading Susan down a path of reducing her anxiety. As Susan

MAJOR DEPRESSIVE DISORDER 5 continued to self-monitor her glucose she learned to enhance her daily diet and lower her A1C readings. With the initial assistance of the Harvard Department of Psychiatry National Depression Screening, Susan’s PCP was able to position her to learn how to make correct choices in everyday life with a small dose of medication and continued medical follow-up. It is possible that Susan initial may have been a victim of Postpartum Depression (PD) that sustained and lead to MDD over the years since her daughter was born. The unfortunate factor was she
