By year three, 100 percent of surgical patients will have an appointment set up prior to discharge for a post hospitalization office visit within 2 weeks of discharge Further implementation of the VBP: Patients who have post-operative complications following high-risk surgery have a significantly lower risk of being readmitted to the hospital within 30 days if they go see their primary care physician soon following discharge [Bro14] . Following up with a physician after discharge from the hospital can aid in early detection of potential adverse effects and poor outcomes Often patients are overwhelmed when they are getting discharged from the hospital due to information overload. Providing an early follow up can help clear up any confusion and provide an opportunity for re-education on self-care, medication administration and other prescribed therapies. B4A.
