CHANGE PROPOSAL 9 Appendix A: Grant Proposal Need Statement The rural area of Jordan is severely lacking healthcare services within proximity of the downtown area. This area is suffering from health disparities due to the poverty status in this area and the closest medical facility is miles away. The opening of a new healthcare facility will deliver care to the community for preventive services, screening, education, and treatment. Lack of healthcare in a proximity to downtown rural area of Jordan can lead to delay in disease diagnosis, preventable disease development, easily spreadable diseases and poor overall health of the community (AAMC, 2017). There is increase in the opioid crisis, spreading preventable disease and lack of access to affordable healthcare. Due to the poverty level in the area, most residents cannot afford traditional insurance and healthcare services which plays a key factor is the increasing morbidity and mortality in this community. The scope of the proposal is to all patients in the rural area of Jordan. About 50% of Jordan’s population is under 30 and 70% is under 40, the median age around 29 of the 6,500 people in Jordan (Capella University, n.d.). The population in this community is at prime age for preventable care and developing comorbidities due to lack of care and screenings. These patients are also part of the stakeholder community that will be included into the development of creating another healthcare facility. This project is designed to decreases health disparities in this community and increase access to all types of medical care. Some barriers to this project are likely financial means, lack of experienced employees and limited time frame to complete and get a medical facility running. The major barrier
CHANGE PROPOSAL 10 to this project is skepticism from the community. This can pose a challenge when building census for clinic staff and patients. This change initiative is consistent with Triple Aim Framework and bring better access to healthcare to rural areas. Using the already establishing initiate started by the US. Department of Health and Human Services to reduce health disparities through healthcare access, partnering with St. Francis Health Center to bring a medical facility that brings affordable care to this neighborhood. Program Description This change initiative will be changing the current level of access to healthcare in the rural area of Jordan, therefore, reducing health disparities and preventable disease. This change proposal will be directed at the communities in poverty and rural areas of Jordan. This initiative is striving to create change and better access to affordable healthcare services.
