Nutrition AAP recommends breast milk or formula as major source of nutrition for first 12 months of life No water or any other liquid except formula/breastmilk until 12 months old o Can’t regulate electrolytes – fluid overload easily  hyponatremic Newborns eat 8-12 times per day and on que o Don’t let them go any longer than 4 hours Older infants: every 3-4 hours Can introduce solids (purees) at 4- 6 months Breastfeeding vs Formula Breastfeeding is best o Supplement with Vitamin D o No iron supplementation is needed for breastfed babies until 6 months o Easier to digest o Maternal antibodies Iron fortified formula second best Some children may need special formulas o Inborn error of metabolism o Digestive difficulties What does breastmilk have that formula does not? o Maternal Antibodies (immunoglobulins) o Especially IgA o WBCs (neutrophils most common) o Omega-3 Fatty Acids, DHA o Highly absorbable iron o Breastfed babies and bowel movements? Yellow mustard & seedy poop Benefit - has little to no odor Breastmilk is free o Cons – work & pumping, experiences, only mom can feed Benefits of Breastfeeding Dose depended – more & longer baby is breastfed = more protection

Preparation of Formula or Breast milk Do not give extra water! o Malnourishment, electrolyte imbalances Mix according to manufacturer’s directions Do not heat in a microwave- scalding Do not bottle prop – let bottle sit in mouth o Aspiration, dental problems, ear infections Do not give bottle in bed Discard one hour after bottle started If giving pumped breastmilk in hospital setting, RN double check and other procedures required o Nurse, tech, pt family member Solid Foods: Assessing for Readiness Extrusion Reflex (tongue thrust) o Stops 4-6 months Head control Sit with support Interest in Parents’ food Solid Foods Rice cereal first , few allergens o Powdered/simple grain – soupy o Cream of wheat – see if interested, can they swallow o DON’T put in bottle – no nutrition, don’t do without doctor order… bad 4-6 months, better to wait until 6 months . Why? o Don’t fill belly with less nutritional stuff o Reflex – spit food out Baby foods (pureed food) may be introduced after rice cereal o Green beans, peas, then fruits Add a new food only every 2-3 days o Looking for reactions to food Fruits or Veggies first? o Doesn’t matter but do veggies first, fruits taste better – reluctant to take veggies after Foods to Avoid Allergenic Foods??? o Used to.
