B1-B1a . Med-Surg (Domain: unplanned hospital visits) The role and function of this department are to provide medical care to a wide array of acutely ill and post-surgical patients. The medical-surgical unit is essential for implementing VBP within the organization by preventing unnecessary re-hospitalizations. Nurses' Role and function are to educate the patient about their healthcare before discharge. Increased coordination of care could reduce the rehospitalization rate, which could increase the reimbursement rate. B2. Goal The nurses will work collaboratively with the interdisciplinary team to ensure that patients are essential ready and appropriate for discharge by the end of year one. Evidenced-based practice incorporated in the discharge process to improve patient education on their medications, self-care, and disease management by the end of year two. The HCAHPS scores for the med-surg unit will be decreased by 10% by the end of year three. B3. Attaining Quality Outcomes in patient care over the next 3 years.
