Clinical Nursing Care: - Provide proficient, evidence-based chronic disease management. - Conduct preventative screening. - Demonstrate organisational values including respect, dignity and cultural safety. - Plan and manage patient care in consultation with medical staff and act as patient advocate. - Coordinate care in collaboration with other providers. - - Share innovative practice and work as an integrated and highly effective member of the primary health care team.
