Phone a full report of the patient’s condition and plan of care to the receiving nurse on the new unit. Document the time of transfer in the nurse’s notes. Transfer the patient with all his or her belongings and chart to the new unit. Transfer to Another Facility After obtaining the physician’s transfer order, notify the business office of impending transfer. Assist the patient in arranging for transportation, such as calling for an ambulance or notifying a family member if the patient can go via private automobile. Some facilities require both a transfer summary form and a discharge summary to be sent to the receiving facility. A transfer summary form is prepared that contains information about the patient’s condition and vital signs at time of discharge; discharge instructions that include a list of the patient’s discharge medications, diet, activity restrictions, patient teaching that has been done, and follow-up appointments; time of transfer; and method of transportation. This form usually is completed by the transferring nurse, and a copy is sent with the patient to the receiving facility. Some facilities may also require the social worker to complete a transfer information record. Another document commonly sent with the patient transferring to another facility is a document called a discharge summary. The physician completes this document, which summarizes the patient’s condition, diseases, pertinent diagnostic results, medications and treatments, as well as where the patient is to go after discharge and follow-up treatment plans. Any valuables in the hospital safe should be sent with the patient and notation made in the hospital chart and the transfer record.
