The salt restrictions are put in place to reduce fluid retention. The Diuretics provide relief of the left ventricle ejection fraction symptoms. Angiotensin (ACEI’s) are used in supporting the vasodilation and neurohormonal modification in the left ventricle ejection fraction, offering survival relief and benefits.

ADVANCED INFORMATION MANAGEMENT AND THE APPLICATIO 7 Angiotensin receptor blockers (ARBs) are used for vasodilation, improvement of the left ventricle ejection fraction, and neurohormonal improvement. Hydralazine and nitrates are used to improve heart function. It’s used to support the ventricular function, exercise capacity, and survival in patients who cannot tolerate the ACEI/ARB, and it can also be added to support the ACEI/ARB and beta-blockers in the African American population to offer benefits for survival. Beta-adrenergic blockers are used for neurohormonal modification, improvement in cardiac symptoms and LVEF, arrhythmia prevention, survival benefits and control of ventricular heart rate. Aldosterone antagonists, functions as an adjunct to other drugs adding in the effect of diuresis, heart failure symptoms and control, improved heart rate, decreased ventricular arrhythmias, decreased cardiac workload, improved LVEF, and increase in survival. Digoxin is used to support the improvement of symptoms. The functions support increase in cardiac output, improvement in heart failure symptoms, and decreased rate of heart failure hospitalizations Anticoagulants are also used to prevent blood clotting and decrease the risk of thromboembolism. Inotropic agents are used to restore organ perfusion and reduce congestion (Dumitru MD & Baker, 2017) The State and National practice standards recognize Heart Failure as a national initiative within the American Heart Association (American Association of Heart Failure Nurses, n.d.). In an effort of supporting the Heart Failure concerns within my organization and at the local level, it calls for a thorough assessment of patients within the population. Utilizing the health care professionals available to support the outreach and plan of care is critical to supporting the overall success of the goal. With the implementation of the new EMR Epic my organization we now have the availability to provide cross-service communication i.e. the Cardiologist may now view the primary care documentation to support the patients plan of care. Both services are now able to provide a better plan of care for the patient and therefore resulting in the patients having improved outcomes of care.
