The manufacturer/ICD-10-PCS recommended code assignment has not yet been determined. Moreover, general guidelines state that “a device is coded only if a device remains after the procedure is completed. If no device remains, the device value No Device is coded [Cas184]. However, because the device is ingested it is in fact remained and the device value should not indicate a value of “Z”. When the new experimental/procedure device develops to fruition, a more appropriate code can be assigned. Coding Procedure The CPT code for this procedure is 0312T. The ICD-10-PCS code is 0JH60CZ. The CPT code is a category III code. Category III codes “are a set of temporary codes for emerging technologies, services, procedures, and service paradigms” [Cat18]. These codes have an alpha character as the 5 th character. The use of this code was found by the procedures indication of “emerging or new technology” and they “describe endovascular procedures performed percutaneously and/or through an open surgical exposure [Ath18]. Compliance The Healthcare Common Procedure Coding System (HCPCS) is a set of health care procedure codes based on the American Medical Association's Current Procedural Terminology. The 3M coder HCPCS coding clinics First Quarter of 2018 presented these guidelines for the Vagus Nerve Stimulator. The Vagus Nervous procedure does not have any codes assigned to its creation. However, there is an unlisted code that has already been created for when CPT codes do not exist. The code 6499- Unlisted procedure, nervous system would be considered the CPT code. HI 223-8 4 HCPCS modifier to this CPT code since the nervous system is not a specific anatomic site nor a designated side (Guidelines PCS, 2018). ICD-10-PCS codes for the Vagus nerve stimulator placed for the treatment of seizures is defined by the 3M Coding Reference-CPT Assistant as “Vagus nerve stimulation (VNS) therapy is a neurostimulator therapy delivered from a small pacemaker-like generator implanted in the chest that sends preprogrammed, intermittent, mild electrical pulses through the vagus nerve in the neck to the brain” [Gui18]. When one approaches the documenting of the procedure through ICD-10PCS, one can follow a logical sequence specific from the vagus nerve itself to the insertion of the rechargeable stimulator SetPoint is using. The resulting code is 0JH60CZ, a billable medical procedure code. Local Medical Review Policy (LMRP) is an administrative and educational tool.
