there were no sores or lumps/bumps noted inside the mouth or on the hard and soft palates. The teeth were clean and intact. Gag reflex intact, tonsils pink. The uvilla rose and fell, tongue
midline and symmetrical. Patient able to push tongue against examiner’s hand on inside of cheekequally. The patient’s ears are pink, warm to touch and elastic. No drainage or redness noted. Patient correctly repeated whispered words during hearing examination. Neck symmetrical, no pain, tenderness or masses noted with palpation of cervical lymph nodes. There was upward movement of the thyroid gland during swallow test; no deformities or masses noted with palpation of thyroid. Carotids equal, no bounding pulses felt, or bruits heard. The patient was able to perform ROM exercises correctly and strength was equal bilaterally. His posterior skin was appropriate for ethnicity, no sores or open areas seen on exam. No pain on palpation of spinal cord. Resonance heard with percussion. Respiratory excursion intact, examiners hands moved symmetrically on inhalation. No pain or tenderness with exam of CVA. Lung sounds clear, no adventitious sounds heard. On the patient’s anterior exam, skin pink, warm and dry, no sores noted. No deformities or pain noted with palpation of chest wall. Patient’s breathing was equal and nonlabored. Percussion reveal resonance sounds and tactile fremitus was equal. Lung sounds clear and equal. During exam of the heart at the aortic valve, the apical pulse was heard at88. PMI intact at 5thintercostal space. S1 and S2 present, no S3 or S4 heard. Inspection and palpation did not reveal pulsations at the precordium. On examination of the abdomen, a large scar was noted at the lower abdomen. Patient stated he had a previous exploratory laparotomy
