Education provided to mother and father regarding the possible side effects of vaccinations (fussiness, fever, loss of appetite, soreness at injection site, and rash with MMR vaccine). Mother advised to alternate liquid acetaminophen every 4 hours and liquid ibuprofen every 6 hours ONLY if child is fussy and uncomfortable. Mother informed to not medicate fever if child is happy and playful and informed on reasons for this. Antipyretics have been shown to blunt the immune response to several vaccine antigens (Saleh, Moody, & Walter, 2016). Anticipatory guidance provided to parents, which included: reviewing diet (16oz of whole milk per day along with appropriate vegetables, fruits, and meats), promoting self-feeding, promoting routines (family time, bedtime, naptime, teeth brushing), limiting screen time, and safety (5 point car seat rear-facing in back seat until age of 2, preventing falls, drowning prevention, sun protection, pets, safety locking cabinets with harmful chemicals, etc.) (Bright Futures, 2019). Mother informed on importance of finding child a dental home since dental caries are the leading cause of chronic disease in childhood (Hiratsuka, Robinson, Greenlee, & Refaat, 2019). Return to office in 1 month for 2 nd dose of Influenza Vaccine and in 3 months for 15-month wellness exam and vaccinations. Pediatric SOAP Note
