Sub-SECTION E: Decide if, to the best of their knowledge, the patient is voluntarily requesting physician assisted suicide. Sub-SECTION F: Determine if the patient is making an informed decision by providing the patient with information from SECTION 8, Sub-SECTION C. Sub-SECTION G : Confirm, in writing, that the patient seeking physician assisted suicide meets the above qualifications. SECTION 10: All information regarding the patient must be compiled by the attending physician before the patient is prescribed the medication. This must include but is not limited to-- Sub-SECTION A: The patient’s 1st and 2nd verbal requests and their written request. Sub-SECTION B: The attending physician's assessment, diagnosis and prognosis of the patient, and the confirmation that the attending physician believes the patient is acting voluntary and uncoerced. Sub-SECTION C: The consulting physician's assessment, diagnosis, and prognosis of the patient, the confirmation that the consulting physician believes the patient is acting voluntary and uncoerced, and their written statement from SECTION 9, Sub- SECTION G. Sub-SECTION D: Written proof that the attending physician offered the patient an opportunity to revoke their request of physician assisted suicide from SECTION 8, Sub- SECTION I. Sub-SECTION E: A copy of the medication prescribed by the attending physician, and the dosage to be taken by the patient. SECTION 11: Only patients who can prove their legal United States citizenship are eligible for physician assisted suicide. Any patient that cannot prove their citizenship should be given alternative treatment options. SECTION 12: No persons shall be held responsible for the death of a qualified individual that participates in physician assisted suicide. This includes but is not limited to-- Sub-SECTION A: The attending and consulting physicians Sub-SECTION B: The pharmacist who fills the prescription Sub-SECTION C: The witnesses to the patient’s written request
