B7 Timeline The purpose and timeline for this measure will be as follows: Engage nurses to work with members of the therapy team to improve the safety and reliability of care provided by developing a standardized post therapy and shift hand-off procedure that is patient-centered by incorporating multiple disciplines. By the end of one year, 90 percent of the facility staff will have incorporated the new process of hand-off and report and will be meeting protocol guidelines By year two, 100 percent of the facility will participate in and follow the protocols for a standardized hand-off. Including new hires.
