Eyes visual acuity is intact perrla ears hearing

Eyes- Visual acuity is intact. PERRLA. Ears- Hearing intact, TM is translucent and mobile.
Nose- nasal mucosa is moist and pink. Throat- pharynx is pink with no exudate or swelling.
Neck- Supple without adenopathy. The trachea is midline position. No thyroid tenderness or
enlargement and no JVD noted.
Skin: Skin is warm and dry, skin turgor no tenting, normal pigmentation. No clubbing noted on
fingernails or toenails.
Cardiovascular: S1&S2&S3 with gallops present. No peripheral edema noted. Bruit present in
right carotid artery. Thrill palpated in the right carotid artery and amplitude was 3+. No thrill
palpated in the left carotid artery and amplitude was 2+. No bruit noted in bilateral femoral
arteries. PMI displaced laterally, less than 3 cm and amplitude was brisk and tapping. Bilateral
brachial arteries no thrill present and amplitude 2+. Bilateral radial arteries no thrill and
amplitude 2+. Bilateral popliteal arteries no thrill and amplitude 1+. Bilateral tibial arteries no
thrill and amplitude 1+. Bilateral dorsal pedis arteries no thrill and 1+. Capillary refill less than
three seconds on all phalanges. Abdominal aorta bruit present. No bruit noted in the iliac or renal
arteries.
Respiratory: Breath sounds present in all areas, with fine crackles in the posterior right and left
lower lobes. Chest is Symmetric and non-tender.
Abdomen: Symmetric, round with no distention or tenderness. Bowel sounds active in all four
quadrants. No masses noted. All quadrants were tympanic with percussion. Abdominal aorta
bruit present.
