Screening tests: yearly eye screen at Mass Ear Nose Throat Associates (Mass ENT) August 2019 with no retinopathy. Sleep study in April 2019 showing severe OSA with recommendation of CPAP. 9. Safety Measures: Patient reports she wears her seat belt while driving and does not text or talk on her telephone. Patient reports she has a fire detectors throughout her home. 10. Immunizations: Pneumococcal polysaccharlde PPV23 (09/20/2013), Tetanus toxold, absorbed (12/29/2005) INFLUENZA Dosage 0.5ml (10/14/2017, 09/18/2018)" 11. Exercise and Leisure: Patient goes for walk with her two dogs in the morning.

SOAP NOTE Page 3 of 8 12. Sleep: Patient reports sleeping difficulties (insomnia) at night, however she is able to nap in the afternoon. Patient’s recent sleep study depicted OSA and patient currently uses a CPAP at night. 13. Diet: Patient reports she drinks a cup of coffee every morning. Patient report carb counting 24 hour recall Dinner 8/21/2019: “1/2 cup of pinto beans and 1/3 cup of rice.” Breakfast 8/21/2019: “once slice of bread.” Lunch 8/21/2019: “1/3 cup of pasta” Family History: Patient report her mother is deceased with a history of aneurysm, brain, hypertension, diabetes mellitus type 2, heart failure, hyperlipidemia, obesity and major depressive disorder. Patient’s father is also deceased with a history of hypertension, myocardial infarction and asthma. Patient has two living brothers and one sister. Patient’s sister has recently been diagnosed with diabetes.
