forms of computer documentation. I will just be there to help get used to it and to help with anyproblems or questions they may be having. I am so excited to help teach the nurses how to use our newcomputer charting, as this will help prevent medication errors, and also help use to see whatmedications the patients have been on in the past, as well as during any past admissions.In addition to the super user training I attended a monthly Filter Meeting. This team consists ofusually the medical director, DON as well as nursing supervisors and any nursing staff, or therapistsinterested in attending. Each month a different topic is chosen that is of high importance to our facility.Again, this month, our main issues our staffing and safety. In the last month we had two patients thatfell, so this was an area of concern, and one medication error.We decided that upon admission, patients who were high risk would wear a yellow arm band, inaddition to yellow socks, and that their chart would have a yellow sticker. A medication review would beconducted with the nurse and physician at least once daily, in order to eliminate any medications thatcould be altering their mental status or contributing to their falls. If patients do fall, a post fallassessment needs to be filled out, in addition to an incident report in order to identify any contributingfactors. With our new electronic medical record, we are hoping this will help eliminate any medicationerrors.In order to write a personal reflection on my scholarly activities, it is important for me to sharethat most of my nursing career was spent as a cardiac, PCU, ICU nurse, with a few years as an RN CaseManager in the ER and ICU. In 2014 I received the horrifying news that my brother had committedsuicide by hanging himself. I had no idea he was hurting, or anything was going on in his life, as I lived inanother state and was wrapped up in my own life, career and family. This event effected this writer somuch that I decided immediately to become a psychiatric nurse. Since that time, I have worked at twopsych facilities, almost five years where I currently work. I have made it my mission to prevent suicide,and to care for those with mental illness and to prevent another unnecessary death.
