Family HistoryPatient states parents are both living, married for 48 years. Father is 69 years old and mother is 65 years old. Both parents are college graduate Hispanics in good health. He has one biological sister who is 43 years old and has been married for 25 years. She has two sons, ages 24 and 13. She is also a collage graduate with no known medical conditions, other than right
GENEALOGY DISCUSSION3breast lumpectomy, benign and L5, S1 discectomy in 2007. Patient states father and mother are active at the YMCA, but do not follow a healthy diet. Sister works out twice a day 5-6 days a week and measures her intake along with her families. Father has a medical history of heart disease (hypertension and hyperlipidemia). He takes Lisinopril and Atorvastatin, doses unknown. States father was diagnosed with the HTN and hyperlipidemia in 2008 after an episode of chest pain while teaching at the local high school. Father is compliant with medication and medical visits. Father has a cardiologist and a primary care physician. Father has a surgical history of right achilles tendon repair. Mother has a medical history of hyperlipidemia, diagnosed in 2010. Currently takes atorvastatin, she is also compliant with her medication and medical visits. She has a surgical history of tubal ligation, complete hysterectomy, and bilateral benign lumpectomies. Paternal medical history is positive for hypertension, hyperlipidemia, hepatitis, diabetes type 2. Paternal grandfather passed away from complications of influenza at the age of 92 years old, paternal grandmother passed away from a pulmonary embolism post right hip fracture repair at the age of 87 years old. The patient’s fatherhas five siblings, three uncles and two aunts. One uncle passed away at the age of two months from SIDS.
