Standardizing the nurse bedside shift report and moving to the bedside will help ensure the handoff from nurse-to-nurse safe and effective. Research shows that when patients are engaged in their health care, it can lead to measurable improvements in safety and quality (U.S. Department of Health & Human Services, 2017b). Rationale for Recommended Change The creation of the five-star quality rating by the Centers for Medicare & Medicaid Services (CMS) provides valuable information to the community of quality care received within the skilled nursing facilities. Within the geographical region in comparison to other facilities, the overall inspection ranking is lower than average. Delivering marginal care means the facility is an injustice to the community. Bedside shift report is an excellent way to build employee teamwork, ownership, and accountability (Baker, 2010). The rationale for recommended change can lead to improvement of patient-centered care and safety. By changing to shift bedside, reporting provides for an opportunity of early identification of issues and any barriers that could result in a decrease in patient satisfaction. By incorporation bedside shift reporting the facility will implement goals set out by Joint Commission’s National Patient Safety Goals that include: 1. improve the effectiveness of communication among caregivers by managing hand-off communications with particular attention to ensuring the opportunity for asking and responding to question and 2. encouraging patients’ active involvement in their own care (Baker, 2010). Patient safety is the number of priorities in healthcare.

CONSULTATIVE CHANGE RECOMMENDATION 14 Measures to Evaluate To determine if a process of change is an effective evaluation must be done. To evaluate the recommendation of change within the organization several things must occur. The first would be for leadership to incorporate weekly employee meeting to allow for positive and negative feedback of the new process. The meetings must address the benchmark goals and evaluate the quality, quantity, and consistency of achieving those goals. Change is difficult, and resistance will occur; leadership must allow for open discussing by staff. The administration must create an environment of positivity along with an evidence-based reason for the change process. New ideas will be presented along with possible changes to the means for improvement. Another method for evaluation would be the angel rounding by leadership. Angel rounding will allow for open dialogue with patients on the change process to determine if the bedside shift reporting is useful in overall patient satisfaction. The national benchmark to measure if this change process is useful would be seen from CMS in the nursing home comparison ratings.
