Common side effects with Omeprazole are diarrhea, constipation, and flatulence. Clarithromycincan cause abdominal pain, diarrhea, nausea, and vomiting. Metronidazole can cause abdominal discomfort, diarrhea, and nausea (IBM Micromedex, 2019). Mike can use an over-the-counter bismuth subsalicylate such as Kaopectate liquid (Woo & Robinson, 2015, p. 502) to help with diarrhea as needed. Mike will be prescribed an antiemetic to treat the nausea and vomiting. He will take Phenergan 25 mg orally every 4-6 hours as needed (IBM Micromedex, 2019). How would the treatment vary if the patient has GERD instead?If Mike was diagnosed with gastroesophageal reflux disease (GERD), there are four main treatment goals. They are to reduce the symptoms, heal esophageal lesions, prevent complications, and prevent relapse (Woo & Robinson, 2015, p. 1024). It would be ideal to know if Mike smokes or drinks and it would be recommended that Mike make lifestyle modifications like dietary changes and cessation of smoking. He also should not use NSAIDs. Mike will be prescribed a proton-pump inhibitors (PPIs) such as Protonix 40 mg by mouth once daily for up to8 weeks (IBM Micromedex, 2019) because PPIs prevent acid production and are a first-line therapy for patients with moderate to severe GERD.
