on board for labor and delivery in our documenting those protocols.

Normally, I take my students to Hillcrest hospital for clinicals. When patients
come into the hospital, they can do what's called “
acting out”.
They act out of
hiding certain people be around them about being allowed into the lock OB
unit during the time there in the hospital.

They sign a document, which is shared with all of the nurse managers on the
varying parts of OB. It is posted behind the locked door. So, the secretary
doesn't leave let them into the unit incidentally. It is part of something shared
with security.

Pregnancy as a Critical Period
• Good nutritional status
– Optimizes maternal health
– ↓ risk of birth defects
– ↓ risk of suboptimal fetal development
– ↓ risk of chronic health problems in offspring
They in an ideal world every woman would enter pregnancy in the best nutritional shape of
her life. If she was in good nutritional status, she would do better as well as the fetus the
offspring doing better.
Pregravid Weight
• Obese:
– Spontaneous abortion, gestational HPN, preeclampsia, gestational diabetes, C/S (+ wound complications), PPH
– ↑ neural tube defects, other congenital anomalies, macrosomia
– Childhood obesity
• Underweight:
– premature delivery, LBW, IUGR
– “programs” for adult heart disease,type II diabetes, hypertension, hyperlipidemia
The problem is, less than half of women come into pregnancy at a normal pre gravid weight.

And because of this, obese women entering
pregnancy are at higher risk for

gestational hypertension

preeclampsia - the most common hypertensive disorder of pregnancy,

gestational diabetes

C-section. And when they have cesarean sections are more likely to have
wound complications such as infections and wound dehiscence.
