What is offered to them on the exchange is based on their household size and income, not employment status. For patients unable to purchase insurance but aren’t eligible for Medicaid, they can use sliding scale community health centers for basic care. (Healthcare coverage options, n.d. ) Switzerland has universal healthcare, meaning that all children, elderly, retired, or unemployed patients have access to care without the fear of medical bankruptcy. Citizens without coverage are automatically assigned to insurance. Everyone pays their premiums and the government has financial assistance for those who cannot afford it. (Reid, 2008) A2A. Coverage of Medications In the United States, what medications are covered and how much the patient pays for them is reliant on what plan they have. There are numerous plans through employers, individually, and the marketplace. There is also drug coverage through Medicare. There are three factors that influence Medicare coverage of medications; federal/state laws, decisions if a medication is covered nationally, and decisions if a medication is covered locally. If they have the original Medicare Part B, “a limited number of prescription drugs are covered under limited conditions.” These covered medications are paid 80% by Medicare and 20% by the patient. (Retiree Insurance, n.d.) Part C and D are prescription coverage plans, but they will cost more

They are paying out of pocket 100% of all services and medications. In Switzerland, patients also must pay 20% of the cost of their medications if they use a brand name. Generic drugs are sold for 20%-50% less than the brand name. There are no financial incentives for pharmacists to sell more expensive medications, so there is no push to drive up the cost for the customer. (Mossialos, et al, 2017 ) A2B. Referral to See a Specialist In the United States many insurance plans, such as HMOs, require a referral for a patient to see a specialist. The specialist must also be within the plan network. A PPO will allow a patient more options to choose from.
