Remember that you are the client’s eyes and ears in the field and they rely on you r observation to determine whether business appears to be legitimate. When you have doubts or uncertain about what you observed explain your answers when necessary. a) Do the equipment and location make sense for business type? If not, provide an explanation. Are you inspecting fitness equipment at a machinery manufacture? Or a daycare in an industrial complex? b) Does the company share space with other businesses? Sharing space means 2 or more companies are located at the same address, usually sharing the same entrance and with access to each other’s equipment/work areas. It is important to identify the name of the other businesses and explain the relationship between the businesses being inspected. Indications of sharing space would include: Multiple businesses listed on the business sign, building, door, window, and/or business card. Discrepancy between signage posted and business name on the business card. The phone is answered using a different business name rather than the one listed on inspection form. Examples of businesses that share space: An advertising agency leasing office space at a marketing firm. A mortgage office with a title company within office. c) Is there any that appears suspicious or business is not what it reports to be? Does the business appear to not operate the type of business as advertised? When answering this question, be truthful and honest with information provided. Do not make your own conclusions or assumptions that business is not legit. Sample questionable situations: You are inspecting a furniture retailer. You are there during normal business hours, but there are no inventory, no customers, no employees seen other than the contact and the contact said they have been in business in 3 years. You are inspecting a clothing store, but you find that there are only electronics (i.e. TV’s, Cell Phones, and household products) in the store.

Genuinsight Equipment Inspection Guidelines 9 Photographs Confirm that you took photographs of location/equipment. If you did not take any, respond “NO” and provide an explanation. Photos are ALWAYS required even if client select photos are not required. We at least expect you to take photos of business location. If no access to location explain why and take photos of surrounding area. Verification of Equipment The inspection of the equipment will cover the following details: 1. Did you see the item? If not, why? 2. If you saw the item: a) Provide status of equipment. b) When provided, is there a discrepancy in Serial/VIN. c) Obtain Serial/VIN viewed on equipment. d) When provided, verify/obtain the make/model/if applicable, license plate information.
