No pain or tenderness noted with palpation. Otoscope exams reveals no cerumen, no lesions, discharge or foreign bodies. TMs pearly-grey with bony landmarks and light reflex visible. No bulging or retractions noted. No fluid or air bubbles visualized. Whisper, Rinne and Weber intact. + PhonophobiaNose: Symmetrical and nares patent bilaterally. No rhinorrhea, crusting or flaring noted. Turbinates pink bilaterally; septum midline without perforation. + tenderness with palpation. + sinus tenderness with palpation. Throat/Mouth: All teeth present with a couple of fillings noted. No missing or chipped teeth. No dental appliances. Lips are symmetrical and pink. They are smooth, without lesions or nodules. Tongue is red without crusting. Oral mucosa pink and moist. Buccal pouch without nodules or lesions. Oropharynx without nodules or masses. Tonsils grade II. No redness, edema, or exudateNeck: Supple, full ROM. + tenderness with palpation. Thyroid ascends with swallow test. No nodules or masses palpated. No lymphadenopathy. Trachea midline. No carotid bruits heard with auscultation. No JVD. JVP 7 cm at 45-degree elevation. No webbing or skin-folds noted. Chest: Breathing appears non-labored and symmetrical.
