o Placenta Abruption (Abruptio) Risk Factors include abdominal trauma, HTN, previous abruption, PPROM, Cocaine use & cigarette smoking. o Maternal Risks hemorrhage/shock, DIC, hypoxic damage to kidneys/liver, PP hemorrhage. o Fetal Risks preterm birth, hypoxia, anoxia, IUGR, fetal/neonatal death o Placenta Abruptio is a Surgical Emergency in order to save the fetus and stop maternal hemorrhage. o Ingestion of the drug Cocaine can cause a placenta to abrupt within 20 minutes. Types of Abortion o Abortion (AB) the spontaneous or elective termination of pregnancy before 20 weeks gestation. Abortions are categorized as induced, elective, therapeutic & spontaneous. o Induced AB the medical or surgical termination of pregnancy before fetal viability. o Elective AB termination of pregnancy before fetal viability at the request of the woman but not for reasons of impaired health of the mother/fetus (TOP, VIP)— termination is done transcervically through dilation of the cervix, then evacuation of the uterus is by mechanical curettage, scraping of the uterine contents or by vacuum suction. (D&C) o Therapeutic AB termination of pregnancy for serious maternal medical indications or serious fetal anomalies. Got a diagnosis for down syndrome. o Spontaneous AB abortion occurring without medical or mechanical means (aka miscarriage) hemorrhage in the decidua basalis is usually followed by necrosis of tissue. Risk Factors for Spontaneous Abortion o Increased parity – more than one pregnancy o Increased maternal/paternal age o DM o Environmental toxins o Drug use (illicit & OTC) o Autoimmune disease o Systemic disorders o Genetic factors o Uterine or cervical abnormalities o Past medically induced Abs Signs & Symptoms of Abortion o uterine bleeding followed by abdominal cramping o US confirms diagnosis Classifications of Abortion

o Threatened AB vaginal bleeding/spotting w/ mild abdominal cramps in the 1st half of pregnancy cervix closed, uterus soft, non-tender & uterine enlargement is appropriate for gestational age no effective therapy other than bed rest to diminish uterine irritability. o Inevitable AB termination of the pregnancy is in progress cervix is dilated, ROM, vaginal bleeding w/mild to moderately painful UCs. ROM – rupture of membrane o Incomplete AB fragments of the products of conception expelled with parts remaining in the uterus profuse bleeding occurs r/t retained products of conception. o Complete AB products of conception are completely expelled from uterus minimal vaginal bleeding is generally evident. o Missed AB Embryo/fetus dies within 20 weeks gestation & is retained in the uterus for 4 weeks or more afterward r/t intermittent vaginal bleeding, spotting or brownish discharge baby No heartbeat.
