PHARMACY REVIEW 5 have the prescription filled at the outpatient pharmacy at our hospital allows the patient to go home with their prescriptions and not having to go pick them up. This ensures the patient actually has the medications they need and are more likely to take their medication. This helps reduce readmittance to the hospital. This feature is called Meds-to-Beds. In some of the research I did on Meds-to-Beds, the program reduced hospital readmission by 50% (Aston, 2016). This program provides opportunities for the discharging nurse to provide proper education and answer any questions the patient may have. The patient is also able to speak with the pharmacist if they have questions the nurse cannot answer. Any potential barriers such as lack of transportation, high copayments, and low literacy can be eliminated with this program (Gardenier, Le, & Baker,
