(McCormack & McCance, 2017). The macro context consists of political and strategic influences such as, policies, frameworks and leadership (McCormack & McCance, 2017). Prerequisites outlines the required nurse attributes including competence, interpersonal skills, commitment, clarity on beliefs and values and self-knowledge (Li, & Porock, 2014). The care environment focuses on the delivery of care context which includes, relationships of staff, support from organization, sharing of power, an appropriate mix of skills, innovation and taking risks (Li, & Porock, 2014). The PCN framework underpins the concept of “person-centredness,” a new PCC model that involves a more holistic approach to healthcare centered around fostering relationships between healthcare providers, patients, and those significant in their lives McCormack & McCance, 2017). The patient-centred process (PCP) consists of care delivered through shared decision making, being sympathetically present, taking into consideration the beliefs and values of patients and tending to their physical and emotional needs (Li, & Porock, 2014). When done effectively, outcomes include patient satisfaction, a therapeutic environment, patient involvement in care, and overall welfare (Li, & Porock, 2014). The nursing metaparadigm concepts; person, health, nursing, and environment, are all addressed by the PCN framework and align well within the theory’s constructs. The frameworks pictorial representation incorporates all constructs of the framework and a specific order must be followed to reach effective care; starting with the outermost, macro context, and works inwards toward the person-centred outcomes (McCormack et al., 2015).

Furthermore, person-centered care is dependent upon creating good therapeutic relationships; easily established through continuity of care in FNP practice. The role of FNP is especially significant
