Adequate nurse staffing is pertinent to the delivery of safe, cost effective, quality, care that impacts patient outcomes. Patient to nurse ratio disproportions have gained the attention of state legislatures. State staffing laws have been initiated in 18 states to ensure safe nursing and patient care; however, Georgia is not one of those states. Evidence has shown that lack of effective staffing takes a toll on the nurse, the patient, and the organizations effectiveness (Karlene Kerfoot, 2012). The ANA has developed and disseminated principles that determine nurse staffing solutions through evidence-based research (ANA,2012). Decreasing the patient to nurse ratios would directly address the weaknesses Supervisor Waltower discussed, by attacking the source. This would inversely increase patient satisfaction scores, improve patient outcomes, increase quality of care, increase safety, improve morale, and decrease nurse turnover rates.

Determination of appropriate nurse staffing is a complex, multifaceted, but it can be solved (ANA, 2012). Therefore, the recommendation is to establish a staffing committee consisting of mostly direct care nurses that can tightly adhere to staffing recommendations. This committee will develop, oversee and evaluate a nurse staffing plan for each nursing unit of the hospital. The staffing plans must be based on available staff and skills, patient severity and needs, and national standards. Patient to nurseratios should not exceed 6:1 on general health units, 4:1 on progressive care units, and 2:1 on intensive care units. The ANA believes that staffing decisions should be based on the care environment and the degree of stability, intensity, and the acuity of the patient (2012). The ANA supports a legislative model that empowers nurses to create staffing plans based on each unit to establish minimum adjustable staffing levels (Lippincott Solutions, 2016). In addition to promoting flexible staffing plans, the ANA and like-minded groups support public reporting of staffing data to promote transparency and penalizing institutions that fail to comply with minimal safe staffing standards (Lippincott Solutions, 2016).Hospital consumer assessment of healthcare providers and systems (HCAHPS) is a national benchmark patient survey developed by the Centers for Medicare & Medicaid Services (CMS) partnered with the Agency for Healthcare Research and Quality (AHRQ), another agency in the federal Department of Health and Human Services (2017). Patient satisfaction rounds can measure short term results of the change while quarterly HCAHPS survey results can be used to evaluate long-term effectiveness of the recommended change. An increase in staff responsiveness which is currently 63%, 4% below state average, and 7% below the national average, is expected following the decreased patient to nurse ratios. Nurses and care staff would be more available to the patients.Quality Improvement projects utilizing Six Sigma techniques was initiated by this organization to reduce turnaround times and increase profits, patient satisfaction, and efficiency.
