One elderly patient that was seen was one who was new, had severalcomorbidities and medications, and required many specialty referrals but did not have aprimary care provider.She came into the office to establish primary care to restart hermedical care.I assisted the patient in contacting her previous PCP for her medicalrecords, get her connected with the required referrals with confirmed appointments, aswell as perform a head to toe assessment and address her chief complaint.Much timewas spent with this patient, and she departed with a new care plan that tackled many ofher concerns in addition to a treatment plan for her chief complaint.Chamberlain Program OutcomesPO#4 (Professional Identity):This outcome on professional identity was achieved bycultivating my own FNP identity through practicing the professional values ofcompassion, trustworthiness, and accountability to my current practice.An example ofexposure to this outcome during this session occurred with a young female adult who hadrecently lost one of her siblings with possible depression.
