accomplished through use of peer-reviewed resources, usage of national guidelines,
distribution of research, and the use of other professional perspectives and clinical
judgement when creating treatment plans. An example is demonstrated when I applied
clinical practice guidelines to real practice is when my preceptor and I referred to the
ACC/AHA and JNC 8 guidelines when we had a patient with acute coronary syndrome.
We created an appropriate treatment plan that best suited the patient using the most recent

evidence-based research and clinical practice guidelines from reputable health
organizations. It is using these very clinical practice guidelines to enhance medical
practice that accomplishes integrating scholarship in primary care settings.
NONPF Core Competencies
Core Competency # 7 (Health Delivery Systems) : Professional identity competency
referring specifically to health delivery systems is encompassed by this core involving the
application of knowledge created from the organizational structure of the healthcare
system and the partnerships related to the continuity of care to impact healthcare. I
provided continuity and cultural competence as a primary care provider during my
clinical rotations. One elderly patient that was seen was one who was new, had several
comorbidities and medications, and required many specialty referrals but did not have a
primary care provider. She came into the office to establish primary care to restart her
medical care. I assisted the patient in contacting her previous PCP for her medical
records, get her connected with the required referrals with confirmed appointments, as
well as perform a head to toe assessment and address her chief complaint. Much time
was spent with this patient, and she departed with a new care plan that tackled many of
her concerns in addition to a treatment plan for her chief complaint.
Chamberlain Program Outcomes
PO#4 (Professional Identity): This outcome on professional identity was achieved by
cultivating my own FNP identity through practicing the professional values of
compassion, trustworthiness, and accountability to my current practice. An example of
exposure to this outcome during this session occurred with a young female adult who had
recently lost one of her siblings with possible depression. She presented to the office

crying and exhausted. It was crucial that she felt safe to openly voice her anxieties and
feelings, which I feel as if I was able to do well. To have the ability to offer a safe space
and enough time for her to express her concerns and then be able to ask questions about
possible treatment plans and resources that are available within our community was
paramount as part of her care plan.
