Disadvantages: spotting & amenorrhea; weight gain, depression, decrease bone density for long-term use (black box warning) Caution: two urine HCG test 2 wk apart or start on their menstrual cycle Intrauteri ne progestin Mirena IUD releases 20 mcg of levonorgestrel daily, left in place for 5yr; minimal systemic ADRs & changes in menstrual bleeding or amenorrhea Progestin implants Implanon one implantable rod that contains 68 mg of etonogestrel and provides contraception for up to 3 yr Emergency Contra- ception Emergency contraception should be implemented as soon as possible (less than 72 hours) after unprotected intercourse; May be initiated up to 120 hours after Method: Plan B One-step & Next Choice (two pills of .75 mg levonorgestrel); a single 30mg of EC (ella) Contraindication only in pregnancy Monitorin g Routine female screening Detailed personal & family histories taking Breast (every 3 yr for female 20-39 y/o) & pap smear (every 2 yr after 21 y/o) Pelvic exam & STIs BP Specific to contraception: BP and history Monitoring for ADRs at 3 mo, then annually abnormal vaginal bleeding; hypertension; amenorrhea; unilateral numbness, weakness, or tingling, indicating possible cerebral spasm or occlusion; breast pain or mass; leg pain; chest pain; sudden loss of vision from possible thromboembolic phenomena; and jaundice Screen for tobacco (> age of 35) & alcohol consumption Interaction effects w/ OC: DM, Sz disorder, migraine HA (need frequent monitoring visits) Education 4 wk before & 2 wk after major surgery, stop combined hormonal contraception to prevent thrombus formation
