As a result, staff will be more confident as they develop and refine their communication techniques with patients and families (Carr, 2017).In review of quality indicator for all cause readmissions, the nurse leader identifies an opportunity to reduce readmission in the heart failure patient population. To improve quality on the unit, the nurse leader will implement a process that involves the nurse case manager to schedule a 5 day follow-up appointment for all patients hospitalized with a principle problem of heart failure prior to discharge. Early follow-up after a heart failure admission is linked to better patient outcomes, reduced mortality rate, and reduces 30 day readmission rates (McAlister, Youngson, Kaul, & Ezekowitz 2016). This will increase patient satisfaction through fewer hospitalizations and improve the trust the community has in the organization through improved outcomes. Improving readmission rates for heart failure population will decrease Centers for Medicare and Medicaid (CMS) penalties, private insurance denials, and increase reimbursement. Creating a culture of patient safety improves outcomes. The nurse leader identified an opportunity to decrease the unit’s fall rates by implementing a Fall Committee. The committee will be responsible for identification of those at risk for falls, better communication of falls and prevention of falls, and this will increase staff accountability for their role in fall prevention. Improving fall prevention and preventing falls will decrease the hospital length of stay (Porter, Culler, Farrington, Matthews, & Tucker, 2018). Falls are considered a non-pay event by CMS, improving falls will improve quality outcomes, reduce mortality and improve reimbursement. Communication of fall risk and prevention with patients, family, and the interdisciplinary team will increase patient satisfaction, and increase team work amongst the interdisciplinary team.

Employee engagement impacts patient satisfaction, quality of care, and safety. The goal is to increase the number of case managers holding the ACM national certification. The nurse leader will implement a plan for the unit to cover the expenses of the exam and recertification. This will improve employee engagement as they will feel the unit places value on continuing education and professional growth. Evidence supports the link between national certification and improved patient satisfaction, patient outcomes, and safety (Boyle, 2017). Improved patient satisfaction, outcomes and safety will increase the trust the community has in the organization and improve overall reimbursement the facility will receive.
