P a g e | 13 Activity 2A Estimated Time 30 Minutes Objective To provide you with an opportunity to review and measure effectiveness of responses in meeting the person’s needs and preferences, in consultation with the person, family and/or carer and/or relevant other and identify and take action to improve areas of the responses that have not met the person’s needs and preferences. Activity Detail four ways of assessing the effectiveness of responses in meeting the person’s needs and preferences. Surveys Interviews Focus groups Reviews of patient notes Describe the ways in which members of your organisation are expected to improve areas of the responses that have not met the person’s needs and preferences. Another way of gauging the effectiveness of support is to ask the carers how they would act in hypothetical situations. You should establish the appropriate responses to such situations and ensure that the care staff are aware of their responsibilities. Changes may be suggested for satisfying the needs and preferences of the disabled individual. It will also be necessary to update the care plan in accordance with any changing circumstances. You should consider these criteria during the review: Progress towards goals identified in the care plan Changes in the physical or mental health of the individual The effectiveness of measures that are already being taken The need to implement changes in line with the person’s needs and preferences There should be a process of continuous improvement in the care environment. You should carry out regular assessments to find out whether the individual’s needs and preferences are being met. Then consider any records that have been taken during the assessment. Reflect and consider the means of achieving improved person-centred practice. Prioritise your care objectives and come up with an action plan for improved care. Then evaluate any measures that have been taken and identify the need for further changes.
