The organization provides care for geriatric patients, particularly for low-income families with a target age group of 65 years of age and older. While the organization is housed in the Richmond area, there are currently no mileage limitations for where a patient must originate in order to become a patient. While some patients need the basic nursing care that may be provided in a nursing home, others are total care patients. According to Nurse Jackie, hospice care is also provided to patients and their families as needed. Due to the small and intimate setting, there are only five patients being cared for at this time in Nurse Jackie’s home; three of which are African American and two are of European descent. She along with one Licensed Practical Nurse (LPN) and three Certified Nurse Aides (CNA) are currently working in her facility. Primary Needs of the Population Served According to the World Population Review, the City of Richmond stretches across 62.5 square miles with a total population of 228,783 people (Richmond, Virginia Population, 2019). Of this culturally ethnic and diverse population, 48.2% are Black, 44.7% are White, 6.5% are Hispanic and 2.1% are Asian (Citation). Over 13% of this population are 65 years of age or older. There are major health disparities that are present especially for the low-income and minority population of Richmond, VA. Barriers to obtaining proper healthcare are transportation, the lack of knowledge of resources, familial support and mental health conditions that seem to be on a rise. According to Nurse Jackie, the primary needs of the population she has served in her facility include; but are not limited to, diabetes and hypertension management, obesity, chronic illness such as kidney failure, heart disease, stroke, paralysis, Dementia, Alzheimer’s, mental health conditions such as depression. Role of the Nurse Leader One of the important roles of an advanced practice nurse is to find ways to continuously improve the quality of healthcare and related processes. This is achieved by examining, evaluating, documenting and designing process for innovation in healthcare. As the owner and Nurse Leader of Grandma’s Hands, LLC, Nurse Jackie has a great deal of responsibility to ensure her facility meets state standards for Senior Home Care facilities and that she provides her patients with the best care possible. It is her job to ensure workplace safety, advocate for her patients and implement any necessary changes to promote a safe environment for her patients and employees. Another responsibility is ensuring quality patient care. According to Patient Safety and Quality Healthcare, “As healthcare focuses on an increasing number of safety initiatives, nurse leaders are key to successful outcomes” ( Schmidt, Papa, Rich, & Cen).
