Outcomes: The expected outcome is an improvement in knowledge of stroke S/S and mNIHSS, improved competency and compliance with the use of mNIHSS, improved documentation that meets TJC standards, and improved quality and assessment of stroke for new and established nurses. Plan for Sustainability It is important to monitor and sustain the processes and outcomes as a way of ensuring there is continued long-term improvements in the care of stroke patients. Train- the-Trainer (classes or sessions) will be done to educate two stroke certified nurses to continue the education and disseminate system wide. Additionally, ongoing chart reviews will help identify deficiency in compliance, and re-education as needed utilizing the CBL education previously developed followed by mandatory recertification on a yearly basis. In same manner, the implementation of the tools into EHR promotes long-term sustainability as it will be a permanent part of the EHR
