Reflective Journal Health care delivery and clinical system encompasses the management and delivery of quality and safe health services so that people receive a continuum of health promotion, disease prevention, diagnosis, treatment, disease-management, rehabilitation and palliative care services, through the different levels and sites of care within the health system, and according to their needs throughout the life course. This week my mentor and are speaking to healthcare staff on our floor including nurses, doctors, pharmacists, etc. After discussions with the shared governance committee about patient fall rates, and implications, we want to introduce hourly rounding as an identified evidenced based practice to reducing the falls rate and ask for their cooperation and support to making this evidence based practice effective and successful. Week 4 This week my mentor and I continued to work on gaining the full support of all the interprofessional team, most importantly nurses to making our project successful. Again, the shared governance meeting was held to discuss the response of the people on the identified evidenced based project. We had a positive response overall as most if not all of the interprofessional team involved are willing to go the extra mile to ensure that this evidenced based practice is effectively done and continued even after the completion of this assignment to ensure patient safety. I am excited to have my team members involved, and looking forward to a successful completion. Week 5 Ethical consideration is also an important part of healthcare policy, so as we have began practicing this evidence-based practice effectively as a team. It is important that we seek the approval of our patients to round on them hourly/purposefully to ensure their safety. A meeting will be held at the beginning and end of each shift to continue to remind the team to inform patients at the beginning of the shift of our plan of rounding on them hourly/purposeful and 3
