Organizational Culture Santa Rosa’s Children’s Outpatient Clinics were established with a mission to extend the healing ministry of Jesus Christ[ CITATION CHR20 \l 1033 ]. Delivering quality care to those that need it most. Children with chronic UTI’s have a higher risk of complications that can lead to end stage renal disease (ERSD) . In addition, it plays a large role in psychosocial and quality of life for these children . Research shows that early diagnosis and treatment can reap improved patient outcomes, by reducing secondary comorbidities ( Santos, Lopes, & Koyle 2017). An assessment will be conducted to appraise organizational readiness to intake changes and policies. Addressing concerns and questions in real time to develop trust and get input from those involved. 2

EVIDENCE-BASED PRACTICE PROPOSAL – SECTION D Expected Outcomes Expected outcomes would include a reduction in chronic UTI symptoms and or infections when constipation is controlled. Additionally, the same goals would exist for patients and study, to develop a method to identify safe, quality care by the implementation of the new assessment questions.
