9 form of thyroxine that is the converted into an active form T3. These then bind to receptors to exert metabolic affects. This medication can take 4-6 weeks to see thyroid levels stabilize. Benzonatate is an antitussive that helps to suppress coughs by topical aesthetic action. The indication for this medication is the patient complaining of cough and Robitussin is not working. The dosage of 100mg is a starting dose for the patient and can be increased to 200mg three times a day if needed. The patient is also a diabetic and a lot of syrups have sugar in them, which can increase blood sugar. If the patient uses these medications as prescribed his response should be good. He should come back in one month and get new blood work to reevaluate and make sure they are working appropriately. Most of these medications are metabolized in the liver and excreted in the urine. His kidney function is not bad, which means he will be able to excrete medications appropriately. Hepatic function is not shown in the blood work so it should be checked. As long as blood work comes back within normal limits there should be no issue with metabolizing these medications. Correct Dosages Based on the patient’s diagnosis we ordered several medications to treat them. First, the patient has stage II hypertension. To treat hypertension we prescribed hydrochlorothiazide 25 mg tablets orally. According to Epocrates the dosage for HTN is from 12.5 to 25 mg oral daily (2019). Since the patient has no problem swallowing the oral route is the preferred one for this and all medications. Hydrochlorothiazide is a thiazide diuretic, is used to treat HTN. The second medication indicated to treat HTN is losartan 25 mg tablets daily. Losartan is an angiotensin II receptor antagonist. It is beneficial for this patient because Losartan is also used to slow long-term kidney damage in people with Type 2 Diabetes, who also have high blood pressure. This is a drug of choice for this patient because he has HTN and Type 2 Diabetes. The

10 most current information indicates that for patients on diuretics the start dose is 25 mg tablets orally. The third medication for this patient is to continue metformin 1000 mg tablets twice a day. Metformin is used together with diet and exercise to improve blood sugar control in adults with type 2 Diabetes Mellitus. The patient’s dose was increased a month ago help lower his blood sugar. The maximum dose for metformin is 2550 mg/day, according to Epocrates (2019). Fourth medication for this patient is Dapagliflozin 5 mg tablets orally daily in the morning.
