Transition Plan Template
The Transition Plan is used to describe how the changes to the business unit(s) environment will be implemented.
The Implementation Plan focuses on changes that are anticipated, planned ahead of time (eg. a reduction in the
number of staff). However, other changes may result, such as those that emerge and are unintended (eg increased
salaries). At a minimum, these changes or issues will require tracking.
