O. Additional Course Requirements and Policies 1. Attendance Requirements In the case of an anticipated or unanticipated absence, please call or email the Instructor to arrange a make-up plan. Email notification of the expected absence is essential for documentation purposes. If the student fails to inform the Instructor of the absence, it will be counted as an “unexcused absence” and will result in zero attendance points for that session. Students who are absent from their team’s in-class Policy Analysis Presentation will receive a 5-point deduction on their grade for that assignment, since teamwork is an important aspect of this assignment. 2. Late Work Policies For assignments, 1 point will be deducted for each day past due, (e.g., if the assignment is 3 days late and it was worth 15 points, the maximum amount of points that can be earned will be lowered to 12). 3. Make-Up Work Policies In cases of illness or personal crises, the Instructors may grant permission to make up missed tests or assignments and/or request that the student complete a unique make-up assignment. This will be determined on a case-by-case basis upon consultation with the Instructor. 4. Electronic Submissions 8

Unless otherwise requested, all assignments and reports must be submitted electronically to the Instructor. Failure to do so will result in a 5 point deduction on the grade for that assignment. 5. Extra Credit Extra credit assignments are not allowed for this course. 6. Reference Sources The Instructors strongly encourage students to support their assignments using credible references. Please be advised that citation of unverified Internet sources such as Wikipedia may result in a reduced grade for affected assignment(s). P. Compliance with University Policies 1. This course will adhere to the Code of Conduct and Academic Policies and Procedures (Grading System, Academic Integrity, Confidentiality of Student Records, Disability Accommodations, Add/Drop / Withdrawal, Grade Appeal Protocol, Registration, etc.) as contained in the Jefferson School of Pharmacy Student Handbook. 2. Registration Status: Students are responsible for the status of their registration at all times. The instructor will advise that a change in status has occurred when so informed by the School/University. Only students who are properly registered and who appear on the official class roster can remain in this course and earn credit and receive a grade. 3. Exams administered in this course will comply with the Jefferson School of Pharmacy Exam Policy (posted on Blackboard). Q. Class Cancellations Cancellations due to weather or other emergency will be announced on KYW AM radio (school closing number: day 173, night 2173), on the TJU emergency hotline (1-800-858-8806). 9
