Lifestyle: M.G and his wife operate and own a retirement home facility. He has a business degree and lives with his wife in a ranch home in the suburbs community with little crime. Their two children are in college. Strong family, neighbors and church support system. Patient reports changing his lifestyle behavior and now maintaining a healthy diet, walks around his block for about an hour every morning and goes to the gym to exercise twice per week. Review of Systems: General: Denies fever, fatigue or weakness. Adequate sleep of at least eight hours per day. No significant weight gain or losses. HEENT: No changes in vision or hearing, does not wear glasses, see the eye doctor every two years. His last eye test was a years ago. No glaucoma, diplopia, floaters, excessive tearing, or photophobia. Denies having ear infections, tinnitus, or ear discharge. He reports his sense of smell is intact and denies epistaxis. His last dental exam was six months ago, denies lesions, gingivitis, or gum bleeding. No difficulties with chewing or swallowing. Neck: No pain, injury, or history of degenerative disc disease or compression. Carotids with no bruit, no JVD. Respiratory: No cough, dyspnea at rest or on exertion. Cardiovascular (CV): No chest discomfort, palpitations or murmurs. No history of arrhythmias, orthopnea, paroxysmal nocturnal dyspnea, edema, or claudication. Gastrointestinal (GI): Denies abdominal pain, nausea and vomiting. No changes in bowel or bladder pattern. Patient has a bowel movement every other day. Genitourinary (GU): Denies urgency, dysuria, dribbling, nocturia, straining or changes in frequency. No history of STIs or HPV. Musculoskeletal (MS): No fractures, injuries or trauma. He denies pain or discomfort and has full range of motions. Psychiatric: Denies anxiety or depression, denies history of mental illness, suicidal ideation. Neurological: No syncope or dizziness, no paresthesia. Denies headaches, no changes in memory or thinking patterns. Gait steady, no problem with coordination. No history of seizure.
