There are many different pamphlets available to the patient and the family members, but none are specific to their disease processes. Diversity & Disparities The health system at HMC is a very diverse place to work, as well as receive care. HMC is located very close to downtown Tulsa. This area has a mix of races that seek care from HMC staff. This one of the only secular hospitals in this area of Oklahoma. There are many different languages our patients speak, so HMC has many different types of interpreters available for these individuals (such as language line, sign language interpreters). The policy at HMC does not allow nurse/staff to translate to a non- English speaking patient unless they have passed a competency test in that particular language. Having a more diverse nursing population could be more desirable for this patient. Having more diverse staff can improve patient comfort and trust levels tremendously. Charting and Documentation Patients have the ability to access their online health records and test results online, as well as a mobile app designed for them. Patients and their family members might request certain items from the electronic medical record that are printed. They can do so once the chart has been closed and processed my medical records, they do not have access to what the nurses caring for them document. Care Support HMC patients are at the center of everything. This is why they are asked to update their medications and disease process’s on every visiting or admission to the hospital Family members are asked to be involved in bedside rounding. On the oncology unit family members have restricted visiting hours 7am-9pm. This can be a negative thing because sometimes patients are willing to participate in their care more when their family is with them. Care Goal setting starts when the patient is admitted to the hospital. Everyone from the multidisciplinary team sets goals that can be reached every 24 Nurse asks the patients what their pain level is. Family members give input but sometimes nurses will disregard the input because pain is
ORGANIZED LEADERSHIP 7 hours. This helps the staff give the best possible care. The nurse case manager and/or social worker will arrange the optimal post hospital care. subjective. Weakness A great area for improvement at Hillcrest Medical Center is the leadership and operations department. This department can hold the staff liable for the care that is delivered. Leadership should consistently emphasize promoting patient-centered care and provide tools for measuring the implementation of care. Currently at HMC there are no current policies or procedures that can assure the patient will receive patient-centered care. There is not a tool that is used to measure patient feedback and satisfaction.
