The Agency for Healthcare Research and Quality (AHRQ) has developed the Communication and Optimal Resolution (CANDOR) toolkit to assist organizations in the implementation of communication-and-response programs to assist in open communication between patients and their providers when medical errors occur (Agency for Healthcare Research and Quality, 2016). This program consists of eight modules including PowerPoint slides, facilitator notes, tools, resources, and videos that can be utilized by organizations or practitioners to assist in the timely, thorough response to unexpected events that can cause patient harm (AHRQ, 2016). Many organizations such as Joint Commission and the National Quality Forum endorse full disclosure and call for organizations to integrate risk management programs as well as training for providers and support staff regarding patient safety and its impact to their organizations (U.S. Department of Health and Human Services, 2016).
