colon polyps removed 5 years ago. No recurrent lesions. History of hypertension doing well without medication. Hypercholesterolemia, hypertriglyceridemia. History of mild prostatic hypertrophy. PAST SURGICAL HISTORY: Colonoscopy x2, last year and 5 years ago. MEDICATION: Multivitamin daily and Zyloprim 200mg daily. ALLERGIES: None. FAMILY HISTORY: Known. Negative. SOCIAL HISTORY: Married with three children alive and well. PREVIOUS ANESTHESIA COMPLICATIONS: None. PREVIOUS TRANSFUSIONS: None. REVIEW OF SYSTEMS : HEAD AND NECK: No complaints. CARDIOVASCULAR: No complaints. RESPIRATORY: Non- smoker. GASTROINTESTINAL OCCASIONAL LINE INTAKE. GENITOURINARY: As above, no complaints. RENAL STONES: No prior history. MUSCULAR-SKELTETAL: No complaints. NEUROLOGIC: No complaints. PSYCHIATRIC: No complaints. ENDOCRINE HISTORY OF GOUT: PHYSICAL EXAMINATION: Vital signs temperature 36.7. Pulse 64. Respiration 20. Blood pressure 134/76. Pulse oximetry 94%. Patient does have oxygen on right now. HEENT: Eyes Perla fundoscopic WNL. EARS: Clear. NOSE: Peyton. THREW UP: Negative. NECK: Supple. No masses. Carotids +1 bilaterally. No brewing. LUNGS: Clear to P&A. HEART: Regular. No members or ectopy. ABDOMEN: Good bowel sounds. Soft no masses or tenderness. Femoral Pulses 1+ bilaterally. No inguinal lymphadenopathy. No coastal vertebral angles tenderness. GENERAL RENTAL: Normal. Male genitalia with both testes descended.
