NURSES NOTES FOR LONG TERM CARE CLINICAL
For this long-term care clinical, we are having you write out your assessment findings in the form of a narrative nurse’s
note.
We have provided some samples of assessments. We have also provided a worksheet that you may use to take
into a patient’s room to take notes during your assessment. Record your vital signs and type your physical assessment
findings. This form will expand to fit your typing.
A sample of charting for a long term care resident follows below.

Resident laying in supine position in bed. Alert and oriented times 3. Welled groomed, no visible bumps
or bruises. Hair is nicely groomed no visible infestation. Detailed orient and displays intact memory. Facial
symmetry is equal. Pupils are equal, reactive and accommodative to light 3:1. Oral mucosa is moist and
pink, no indication of chewing or swallowing difficulties. Portion of general diet has been taken at
breakfast. Skin appears to be intact free of any lesions or bruising with elastic turgor<3 seconds, no
noticeable edema in upper or lower extremities. Nasal passage clear mucous membranes are moist and
pink no indication of congestion. Carotid and radial pulse are equal.
Upper and lower extremities equal in
strength full ROM/ capillary refills <3sec. No involuntary movement or abnormal posture noted. Radial
pulse rate 72bpm. Bowel sounds active x4. Abdomen soft, non-distended, non-tender. Visible scar tissue
noted on LLQ extending to ULQ of abdomen. Last Bowel movement this morning, passing of soft frim
brown stools with appropriate amount of clear yellow urine. Lungs sound clear to auscultation. Denies
numbness or tingling to extremities. Toe nails are nicely groomed no noticeable fungi w/ capillary refill <
3 sec.
No peripheral edema noted, pedal pulses palpable and equal bilaterally.
